A house call is medical consultation performed by a healthcare professionals (Nurse Practitioner, Doctor, Physician Assistant) visiting the home of a patient, instead of the patient visiting the doctor's clinic or hospital.

Mobile Primary Care In the comfort of your home
Annual Wellness Visit
A fully comprehensive exam to review your blood pressure, heart rate, height, weight and body mass index (BMI). Review your current health problems, medical, surgical, family and social histories and all current medications. Conduct a health risk assessment to ensure your health goals are obtained. Referral provided to the lab of choice for blood draws. Annual blood draw covers the essential blood tests to gauge overall well being including a full blood count, cholesterol check, liver and kidney function analysis, vitamin D levels, and a diabetes screen. Referrals to health specialist provided as necessary.
Sick Visit
This type of visit is made when you are having a specific health problem, an exacerbation of chronic health problem and or medication management questions/concerns. Reasons for this type of visit can include: Illness such as the flu, strep throat or an ear infection, and non-emergency injuries, and wound care.
Hospital follow up
A hospital follow-up is an appointment given to you with your primary care provider after a hospital stay. It informs your primary care physician that you have been hospitalized, what events took place during the hospitalization, what diagnoses were made, and what new medications were added to your medication list.
Medical Weight Loss
We offer medical evaluation to assess if GLP1 agonist are appropriate to assist you in your weight loss journey. If so a customized plan to include diet and exercise will be developed to give you the tools needed to optimize your weight and health. GLP-1 agonists are the most effective medications for weight loss. Our current GLP-1 agonists approved for weight loss include Semaglutide and Tirzepatide

Concierce Monthly / Annual Membership fee
$400/month per person or get 1 month free if an annual commitment is made.
A great way to minimize cost. Membership allows you to utilize up to 3 visits a month before incurring additional cost. ($170 for each additional visit)
$200/per visit
This option provides you an option to pay per visit if a monthly membership is outside of your need.

This option benefits patients whom are homebound due to acute or chronic medical conditions. Examples are patients residing in assisted living facilities, those who have been injured and experience considerable difficulty leaving their home. If you are unsure about your circumstances call us to discuss your options.
